About Us:We are a compassionate, patient-centered home care agency proudly serving the Stoneham, MA, area. Our mission is to provide high-quality, personalized care in the comfort of our clients' homes. We seek experienced and dedicated Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our per diem team. Position Summary:As a per diem home care nurse, you will oversee our clients' care in their homes. This is an excellent opportunity for nurses seeking flexibility, autonomy, and meaningful one-on-one patient care interaction. Responsibilities:

• Perform initial and ongoing nursing assessments to determine client needs.
• Develop, implement, and revise personalized care plans
• Provide education and health coaching to clients and caregivers on disease management, medications, and home safety
• Coordinate services and follow-ups with ASAP's ( Aging Service Access Point) as well as family members
• Supervise and evaluate home health aides and ensure plan-of-care compliance
• Maintain accurate and timely clinical documentation following regulatory and agency standards
• Participate in quality assurance activities and support compliance with state and home health regulations

Requirements:

• At least 18 years of age
• Current MA RN or LPN license in good standing
• A driver’s license in good standing.
• Reliable transportation
• Must be able to pass criminal background and professional reference checks
• Strong communication skills, both verbal and written, to facilitate effective interactions with clients
• Ability to respect the confidentiality of patients and families

Minimum of 1 year of clinical nursing experience (home care experience preferred) * CPR certification What do we offer:
